When visiting our office for the first time, please bring the following with you:

1. REFERRAL-from your primary physician (if needed).
2. INSURANCE INFORMATION-if you would like us to bill your insurance.
3. MEDICAL INFORMATION-pertaining to the condition you are seeking help with including:
   a) Doctor's Reports.
   b) Diagnostic Test Results(X-Rays, MRI, Lab Tests, etc.).
   c) List of medications you are currently taking.
   d) List of healthcare providers you are currently seeing or have seen for your condition.

FOLLOWING YOUR CONSULTATION AND EXAMINATION, THE DOCTOR WILL DETERMINE IF CHIROPRACTIC TREATMENT IS APPROPRIATE FOR YOUR CONDITION. IF A POSITIVE DETERMINATION IS MADE, THE DOCTOR WILL OUTLINE TREATMENT OPTIONS AND POSSIBLY START TREATMENT ON THE DAY OF YOUR FIRST VISIT. IF CHIROPRACTIC TREATMENT IS NOT APPROPRIATE, THE DOCTOR WILL REFER YOU TO A PHYSICIAN OR SPECIALIST WHO CAN PROVIDE CARE WHICH IS APPROPRIATE FOR YOUR CONDITION.
